HOH Housing provides safe and affordable supportive housing and related support services for individuals and families with serious health issues (including those living with HIV/AIDS), both onsite and throughout the Hartford area. Hands On Hartford is committed to treating those we serve with dignity and respect and is likewise committed to preserving the confidentiality of each individual’s personal information. 55B Apartments and Scattered-Site Housing
Staff provides a broad array of services including, individual counseling, family support, case management, rental assistance, and support groups for individuals and families living with a variety of medical conditions, including HIV/AIDS, who live in their own apartments.
CT Integrated Healthcare & Housing Neighborhoods (CIHHN)
Program provides housing stabilization services, including case management and patient navigation to those who face medical challenges.
HOH Supportive Services at Partner Housing Sites
Supportive Services for those Living at George Beach Apartments
HOH provides less intensive services (referrals, supportive listening), provided on-site for individuals, some with physical or emotional health challenges, living at an affordable apartment building in Hartford.
Zezzo House Case Management
Staff provide direct case management support to individuals at Zezzo House, another supportive housing program in Hartford.
