Our Supportive Housing Program is designed to help tenants achieve long-term stability.
We provide ongoing, intensive case management services to over 100 tenants in the Greater Hartford area—many of whom are experiencing or at risk of homelessness and facing challenges such as mental health concerns, substance use disorders, and chronic health conditions, such as HIV/AIDS.
By focusing on both practical assistance and emotional well-being, we support tenants in overcoming housing barriers and building a solid foundation for a successful future.

George's Journey to Stability
George, 51, faced the lowest point of his life after a divorce left him homeless, without income, and without support. While battling pneumonia, he was also diagnosed with HIV, and his mental health deteriorated, leading to a cycle of despair and isolation.
With help from his case manager, George found hope. Together, they searched for housing, and when an opportunity arose at one of Hands On Hartford’s supportive housing programs for people living with HIV, which provides on-site case management—George was able to find stability. The included utilities eased his financial stress, while the ongoing support from Hands On Hartford helped him obtain furniture, register for SNAP benefits, and secure transportation for his medical appointments.
Today, George has regained a sense of purpose. He participates in Hands On Hartford's community based activities, enjoys meals at Gather55, and is rebuilding his life with a renewed sense of hope. He often expresses gratitude for how Hands On Hartford has helped him "feel good" about his life again.
