Building Trust and Pathways to Housing Stability

Our Outreach Team works in collaboration with the Homeless Response System to address gaps in street outreach, engagement, and service. We actively seek out individuals who are experiencing unsheltered homelessness, building trust and connecting them with the necessary resources and services that set them on the path to stability.

How We Engage

Our Outreach Team's approach is rooted in empathy and support. We meet people where they are, whether that’s under bridges, in parks, or in other places where individuals are experiencing homelessness. We provide essential resources and work toward long-term housing solutions:

  • Risk Reduction Items: We distribute immediate needs such as tents, sleeping bags, water, cold weather clothing items, and food.
  • Building Trust: Through consistent outreach, we create a foundation of trust to ensure people feel comfortable accepting help and resources.
  • Housing-Focused Case Management: We assist with completing housing enrollment paperwork, obtaining identifying documents, and matching individuals to available housing opportunities.
  • Ongoing Support: Once housed, we continue to provide support to ensure housing stability and prevent future homelessness.

From Homelessness to Hope

John, 53, had been homeless for over a year when he was connected with our Outreach Team. One cold January evening, the team met him at a warming center in Enfield, where he shared that he slept in his car when the warming center was closed.

Over the next several weeks, the Outreach Team built a rapport with John, learning that he was living with HIV and eager to find stable housing. They guided him through the complex process of applying for housing through the Housing Opportunities for Persons with AIDS Program (HOPWA), providing ongoing support and encouragement.

By summer, John was matched with one of Hands On Hartford’s supportive housing programs. In the fall, he moved into his new apartment. Today, John is thriving, staying connected to services, and building a more stable future.
